Forms Based Authentication

Was really excited to see this feature. When I try to enable this feature on the site, I get a access denied message. Any ideas?

Usually this is because you haven't added yourself to the credential store. Make yourself a site collection administrator on the FBA-enabled site then enable the feature.

I noticed that class FBAManagement.FeatureReceiver was omitted from the provided source code. Was it done intentionally?
Also, the form UsersAndRoles.aspx is used in the code behind to redirect to, but is missing from the solution.
......
It looks like sorting by user name on the page with list of users is enabled but is not working, is there a plan to make it work?

This is really a copy of Stacy Draper's solution found here http://www.codeplex.com/fba. Stacy had originally posted it here, but expanded to another project.
